To fill this out electronically, use the "highlighter" function in MS Word. If you are unsure how to do this, simply use any other method of indicating your responses, and please don't worry about messing up the formatting. Then just email it as an attachment to the address indicated at the end of the survey.
If it is not feasible to fill out the survey electronically, please print out the Adobe Acrobat PDF file, fill out the survey on paper, and snail mail it to the address indicated.
