Using the WEB BROWSER VERSIONS of the surveys
...is the easiest and best option for educators and community partners!!
The second best alternative is to use the downloadable MS Word version and email it to the address indicated at the end of the survey. To fill this out electronically, use the "highlighter" function in MS Word. If you are unsure how to do this, simply use any other method of indicating your responses, and please don't worry about messing up the formatting. Then just email it as an attachment to the address indicated at the end of the survey.
If it is not feasible to fill out the survey electronically, please print out the Adobe Acrobat PDF file.
