"The Community Readiness Model is an innovative method for assessing the level of readiness of a community to develop and implement prevention programming. It can be used as both a research tool to assess distribution of levels of readiness across a group of communities or as a tool to guide prevention efforts at the individual community level."


Further description:
They have developed an interview process specifically designed to assess community readiness for change.  It has well developed administration and scoring procedures, can be administered as a (45-60 min) individual interview (with key informants from different community sectors) or a focus group interview, and addresses 6 dimensions of readiness.  Scoring results in crude quantitative scores for community readiness on each of the 6 dimensions, as well as a total score.  The Handbook includes recommendations for different types of intervention to address low scores on each of the 6 dimensions.  The "Handbook" link on that page, which includes everything you need, can be downloaded for free.  Farther down on the page is a list of fees for increasing scope of readiness assessment services.
